About the position

ReDiscover is seeking a Community Support Specialist to join our Adolescent Urgent Care location. For over 50 years, ReDiscover has been a non-profit community mental health center in southeastern Jackson County, Missouri, dedicated to delivering efficient and cost-effective treatment to those we serve. Our diverse team of over 700 employees is passionate about supporting our community, particularly individuals whose lives have been impacted by mental illness or substance use. With more than 29 programs across 20 locations in Lees Summit, Raytown, and Kansas City, we offer a variety of career opportunities for individuals looking to make a meaningful difference in the area. The Behavioral Health Urgent Care Clinic (BHUCC) is the first of its kind in the Kansas City area, providing immediate, same-day access to psychiatric medication for mental health and substance use needs. Our multi-disciplinary team is committed to delivering care to clients every day of the week, including evenings, weekends, and holidays, to meet the needs of both clients and the community. As a Community Support Specialist, your role will involve maintaining and interacting with a caseload of clients, facilitating their achievement of long-range outcomes related to symptom management, employment, and independent living based on their individual needs. You will engage with difficult-to-serve clients by exploring all potential resources to help them gain access and make connections. This may involve removing barriers for clients, assisting them in participating in community or social service programs, referring them to educational or work programs, and helping with transportation issues identified in their treatment plans. Additionally, you will be responsible for collecting and maintaining client information and documenting interactions within ReDiscover's designated electronic medical record system. Collaboration with an internal multi-disciplinary team will be essential to address various aspects of wellness and ensure effective communication regarding the coordination of client care. Responsibilities

  • Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
  • Facilitate each client's achievement of long-range outcomes of symptom management, employment, and independent living based on their needs.
  • Engage difficult-to-serve clients by investigating all potential resources to gain access and make connections.
  • Help remove barriers for clients, including assisting with participation in community or social service programs, referring clients to educational or work programs, and addressing transportation barriers.
  • Collect and maintain information about clients and document interactions within ReDiscover's designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements

  • Availability to work a full-time schedule from Monday - Friday 11:00am-7:30pm.
  • A 4-year degree in a social science field or 4 years of equivalent experience in lieu of a degree.
  • A valid automobile driver's license, appropriate automobile insurance, and a safe, reliable, registered automobile available for use during work.
  • At least two (2) years of clinical experience in direct service provision to children and adolescents in a mental health setting preferred.
  • Knowledge of community resources and health and social service systems in the Kansas City metro area, with skills in establishing working relationships with community partners preferred.
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral, and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word, Excel, and GSuite.

Nice-to-haves

  • Experience working with children and adolescents in a mental health setting.
  • Familiarity with community resources in the Kansas City area.
