About the position

The Community Support Specialist Youth at Compass Health Network plays a crucial role in providing integrated and holistic community support services to children and their families. This position is designed to engage both the child and the parent/caregiver, ensuring that treatment is collaborative and tailored to the unique needs of each family. The specialist will utilize a motivational, stage-matched approach to deliver individualized services based on the strengths, needs, abilities, and preferences identified in collaboration with the client and their family. The focus is on supporting the client and their family in achieving improved functioning and meeting their treatment goals. In this role, the Community Support Specialist will work closely with parents to ensure their active participation in the treatment process, which is essential for achieving positive clinical outcomes. Services will be provided using the System of Care philosophy, emphasizing a holistic, person-centered, and strengths-based perspective. The specialist will incorporate coaching surrounding the Eight Dimensions of Wellness, addressing all domains of wellness, including physical and mental health. The specialist will assist families in completing goals outlined in their Wellness Plans, regularly reviewing and reassessing needs, service choices, and community functioning to update goals and objectives as necessary. The position requires maintaining contact with clients and families during hospitalizations, participating in discharge planning, and communicating with all involved providers to develop aftercare plans. Educational information and skill-building will be provided to clients and families regarding daily living skills, coping strategies, symptom management, and communication/relationships, all aimed at improving overall functioning. The specialist will also develop and implement crisis intervention plans and provide necessary crisis intervention services, including after-hours support. Collaboration with the child and family interagency team members is essential to advocate for the mental health needs of the child and family. The specialist will complete documentation collaboratively with clients and families, ensuring all required documentation is submitted within the designated timeframe according to agency policy. Services are primarily provided in the home based on clinical needs, which may include evenings and weekends. Other duties may be assigned as necessary to support the mission of Compass Health Network.

Responsibilities

  • Provide individualized community support services to children and their families using a motivational, stage-matched approach.

Requirements

  • High School diploma or GED required; Associate's or Bachelor's degree in a human service-related field preferred.
  • At least four years of experience in a relevant field if only a High School diploma is obtained.
  • At least two years of experience required if an Associate's degree is the highest education obtained.
  • At least one year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • Experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency-established minimum automobile coverage are required.
