About the position

The Community Support Specialist at Compass Health Network plays a crucial role in assisting eligible children and their families to achieve or maintain optimal social, psychological, and physical functioning. This position is essential in the planning, securing, coordinating, and monitoring of services from various organizations and personnel on behalf of those served. The specialist will engage in teaching, coaching, and practicing necessary skills to facilitate community-based psychiatric rehabilitation. As part of a rapidly expanding team, the Community Support Specialist will work collaboratively in a supportive environment, ensuring that clients receive the best possible care and support. The role requires maintaining communication between families and service providers, providing referrals, advocacy, and education tailored to meet individual client needs. The position also involves regular assessments and monitoring of treatment plans, ensuring that all documentation is completed in a timely manner, and maintaining client medical records to program standards. Driving is an essential function of this role, as the specialist will need to meet clients and families in various locations, including their homes or other safe environments.

Responsibilities

  • Provide wraparound assessment for service planning
  • Assist in and monitor treatment plan development and complete necessary quarterly reviews
  • Maintain all assigned client medical records to program standards and complete documentation in a timely manner (within 72 hours)
  • Provide contact with families at minimum of twice per month based upon client/family needs, one at least face-to-face in the home
  • If the home is not a safe environment then at another identified location
  • Provide referrals, advocacy, information and education to meet individual needs of clients
  • Monitor families' progress and services they receive from various organizations
  • Maintain communication between the family and service providers
  • Driving is an essential function of the position

Requirements

  • High School diploma or GED required
  • Associate's/Bachelor's/Master's degree in a human service-related field preferred
  • At least four (4) years of experience required if no degree is obtained
  • At least two (2) years of experience required if an Associate's degree is the highest education obtained
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
  • A valid driver's license and agency established minimum automobile coverage required
