About the position

The Community Support Specialist - Youth at Compass Health Network plays a crucial role in bridging the gap between children, families, schools, and external systems. This position is designed to provide a wide range of services tailored to the unique needs of each child and family, ensuring that they receive the support necessary for their mental health and overall well-being. The specialist will initially train in a community-based role before transitioning to school-based functions, contingent upon demonstrating the required clinical and professional competencies. This role requires evening and weekend availability to accommodate the schedules of the families served, emphasizing the importance of flexibility in meeting client needs. As a School-Based Community Support Specialist, you will be responsible for collaborating with families and school staff to develop individualized behavioral health services. This includes acting as a liaison and ambassador for Compass Health Network, providing mental health consultation, and advocating for the mental health needs of children and families. You will assist clients and their families in creating Wellness Plans, focusing on a holistic, person-centered approach that incorporates the Eight Dimensions of Wellness. The role also involves helping families access necessary services and resources, providing educational information, and fostering natural support systems within the community. Driving is an essential function of this position, as you will be required to meet clients in their homes and schools. The ideal candidate will have a strong background in human services, with experience working with individuals facing psychiatric, substance use, and developmental disabilities. This position is not only about providing support but also about inspiring hope and promoting wellness within the community, aligning with Compass Health Network's mission to deliver accessible and comprehensive care.

Responsibilities

  • Act as a liaison and ambassador for Compass Health Network to and between the agency and School Districts
  • Serve as a consultant to school systems' staff, providing mental health consultation and information on a wide range of mental health topics
  • Collaborate with other systems' staff regarding assigned children and their families, participating as a member of the multi-disciplinary team to advocate for the mental health needs of the child/family
  • Provide individualized behavioral health services based on strengths, needs, abilities, and preferences identified collaboratively with client/family, focused on supporting client's movement toward recovery/resiliency goals
  • Assist client/family in the development of the Wellness Plan
  • Provide services from a holistic, person-centered, strengths-based perspective, incorporating coaching surrounding the Eight Dimensions of Wellness; supporting client/family in completion of goals
  • Assist the family with accessing, locating, and arranging services and resources to assist in achieving their goals
  • Provide educational information and skill building to client/family and school staff regarding areas of concern such as daily living skills, education, symptom management, communication/relationships with focus on improving functioning
  • Assist client/family in developing natural support systems including work with family members, school staff, legal guardians, etc.
  • Assist with completion of initial and annual assessments and Wellness Plans as directed by Supervisor
  • Seek culturally competent services and service providers to meet the unique cultural needs of each agency client and family

Requirements

  • High School/GED required
  • Associate's/Bachelor's/Master's degree in a human service-related field preferred
  • At least four (4) years of experience OR a combination of higher education and experience required if a High School/GED is the highest education obtained
  • At least two (2) years of experience OR a combination of higher education and experience required if an Associate's degree is the highest education obtained
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
  • A valid driver's license and agency established minimum automobile coverage required
