About the position

The Community Support Specialist provides integrated behavioral health community support services, engaging both the child and parent/caregiver in learning and practicing skills to meet their goals and increase functioning. This role is essential in ensuring that clients receive the necessary support to improve their mental health and overall well-being. Services are provided when needed, during the day, evening, and may also include weekends. The specialist employs a motivational, stage-matched approach to deliver individualized services based on the strengths, needs, abilities, and preferences identified collaboratively with the client and their family. This focus is aimed at supporting the client and their family in moving toward improved functioning and achieving their treatment goals. In addition to direct service provision, the Community Support Specialist works closely with parents to engage them in the treatment process, which is crucial for achieving better clinical outcomes. The role involves providing services to both the child and the parent/caregiver using a System of Care philosophy, emphasizing a holistic, person-centered, strengths-based perspective. This includes coaching surrounding the Eight Dimensions of Wellness, ensuring attention to each domain of wellness, including physical and mental health. The specialist supports the client and family in completing goals identified on Wellness Plans, regularly reviewing these plans and reassessing needs as necessary. They assist families in accessing, locating, and arranging services and resources to help achieve their goals. Maintaining contact with clients and families during hospitalizations, participating in discharge planning, and communicating with all involved providers to develop aftercare plans are also key responsibilities. The role requires the development and implementation of crisis intervention plans and providing crisis intervention services as needed, even outside of normal working hours. The Community Support Specialist also plays a vital role in developing natural support systems for clients and families, working with family members, school staff, and legal guardians. Participation in and coordination with the child and family interagency team members is essential to advocate for the mental health needs of the child and family. Documentation is a critical aspect of the role, requiring collaboration with clients and families to complete all necessary paperwork within the required timeframe, in accordance with agency policy. Services are primarily provided in the home based on clinical need, including evenings and weekends, and other duties may be assigned as necessary.

Responsibilities

  • Provide integrated behavioral health community support services to children and their families.
  • Engage parents/caregivers in treatment to improve clinical outcomes.
  • Utilize a motivational, stage-matched approach to deliver individualized services.
  • Support clients and families in completing goals identified on Wellness Plans.
  • Assist families in accessing and arranging services and resources.
  • Maintain contact with clients during hospitalizations and participate in discharge planning.
  • Provide educational information and skill-building to clients and families.
  • Develop and implement crisis intervention plans and provide crisis intervention services as needed.
  • Work with families to develop natural support systems.
  • Participate in coordination with interagency team members to advocate for mental health needs.
  • Complete documentation collaboratively with clients and families within required timeframes.

Requirements

  • High School diploma or GED required; Associate's or Bachelor's degree in a human service-related field preferred.
  • At least four years of experience working with individuals with psychiatric, substance use, and/or developmental disabilities if no degree is obtained.
  • At least two years of experience required if an Associate's degree is the highest education obtained.
  • At least one year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • Valid driver's license and agency-established minimum automobile coverage required.
