About the position

The Community Support Specialist Youth at Compass Health Network plays a crucial role in providing integrated, holistic community support services that engage both children and their parents or caregivers. This position is essential in helping clients achieve their treatment goals through a collaborative approach that emphasizes the strengths, needs, abilities, and preferences of the client and their family. The specialist will utilize a motivational, stage-matched approach to deliver individualized services, ensuring that both the child and the family are actively involved in the treatment process. This engagement is vital for improving clinical outcomes and fostering a supportive environment for the client’s development. In this role, the Community Support Specialist will work closely with families to help them access and arrange necessary services and resources, thereby facilitating the achievement of their goals. The specialist will maintain contact with clients and families during hospitalizations, participate in discharge planning, and communicate with all involved providers to develop comprehensive aftercare plans. Educational information and skill-building sessions will be provided to clients and families, focusing on daily living skills, coping strategies, and symptom management, all aimed at enhancing the overall functioning of the client. The position also involves the development and implementation of crisis intervention plans, providing necessary support during and after normal working hours. The specialist will assist families in building natural support systems by collaborating with family members, school staff, and legal guardians. Active participation in interagency team meetings is required to advocate for the mental health needs of the child and family. Documentation of services provided must be completed collaboratively with clients and families, adhering to agency policies and timelines. Services are primarily delivered in the home environment, accommodating the clinical needs of the clients, which may include evening and weekend hours. The Community Support Specialist will also be part of an integrated health team, communicating with various healthcare providers to ensure a comprehensive approach to client wellness. This includes monitoring medication adherence, side effects, and overall efficacy, as well as making referrals to additional services as needed. The role requires a commitment to culturally competent service delivery, ensuring that the unique cultural needs of each client and family are met. Driving is an essential function of this position, necessitating a valid driver's license and appropriate automobile coverage.

Responsibilities

  • Provide individualized community support services to children and their families using a motivational, stage-matched approach.
  • Engage parents in the treatment process to improve clinical outcomes for the child.
  • Utilize a holistic, person-centered, strengths-based perspective in service delivery.
  • Support clients and families in completing goals identified on Wellness Plans and reassess needs as necessary.
  • Assist families in accessing and arranging services and resources to achieve their goals.
  • Maintain contact with clients during hospitalizations and participate in discharge planning.
  • Provide educational information and skill-building regarding daily living skills, coping skills, and symptom management.
  • Develop and implement crisis intervention plans and provide crisis intervention services as needed.
  • Assist clients and families in developing natural support systems by collaborating with family members and other stakeholders.
  • Participate in interagency team meetings to advocate for the mental health needs of clients and families.
  • Complete documentation collaboratively with clients and families within required timeframes.
  • Meet service provision expectations based on client needs and agency guidelines, including evening and weekend services.

Requirements

  • High School diploma or GED required; Associate's or Bachelor's degree in a human service-related field preferred.
  • At least four years of experience working with individuals with psychiatric, substance use, and/or developmental disabilities if no degree is obtained.
  • At least two years of experience required if an Associate's degree is the highest education obtained.
  • At least one year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • A valid driver's license and agency-established minimum automobile coverage are required.
