About the position

The Community Support Specialist plays a crucial role in providing integrated behavioral health community support services, focusing on both the child and their parent or caregiver. This position is essential for helping clients achieve their goals and improve their functioning through a collaborative approach. Services are delivered flexibly, accommodating the needs of clients during the day, evening, and sometimes on weekends. The Community Support Specialist is expected to work closely with families, utilizing a motivational and stage-matched approach to tailor services based on the unique strengths, needs, abilities, and preferences of each client and their family. This role emphasizes the importance of engaging parents in the treatment process, as their involvement is vital for achieving better clinical outcomes. In addition to direct support, the Community Support Specialist is responsible for assisting families in accessing necessary services and resources, maintaining contact during hospitalizations, and participating in discharge planning. They will also provide educational information and skill-building activities to enhance daily living skills, coping strategies, and symptom management. The position requires the development and implementation of crisis intervention plans, as well as ongoing coordination with an integrated health team, which includes various healthcare professionals. Documentation is a critical aspect of this role, requiring timely and accurate completion of all necessary records in accordance with agency policies. The Community Support Specialist will also be involved in advocating for the mental health needs of children and families, ensuring that their voices are heard in treatment planning and service provision. Responsibilities

  • Provide individualized services based on strengths, needs, abilities, and preferences identified collaboratively with client/family.
  • Engage parents in treatment to improve clinical outcomes for the child.
  • Utilize a System of Care philosophy and a holistic, person-centered, strengths-based perspective in service delivery.
  • Support client/family in completing goals identified on Wellness Plans and reassess needs as necessary.
  • Assist families in accessing, locating, and arranging services and resources to achieve their goals.
  • Maintain contact with clients during hospitalizations and participate in discharge planning.
  • Provide educational information and skill-building regarding daily living skills, coping skills, and symptom management.
  • Develop and implement crisis intervention plans and provide crisis intervention services as needed.
  • Assist client/family in developing natural support systems, including collaboration with family members and school staff.
  • Participate in and coordinate with interagency team members to advocate for the mental health needs of the child/family.
  • Complete documentation collaboratively with client and family, ensuring compliance with agency policy and timelines.
  • Meet service provision expectations per client needs and agency guidelines.

Requirements

  • High School diploma or GED required; Associate's, Bachelor's, or Master's degree in a human service-related field preferred.
  • At least four (4) years of experience working with individuals with psychiatric, substance use, and/or developmental disabilities if no degree is obtained.
  • At least two (2) years of experience required if an Associate's degree is the highest education obtained.
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves
