About the position

The Community Support Specialist provides integrated behavioral health community support services to children and their families, focusing on skill development and goal achievement. This role involves working collaboratively with clients and their caregivers to enhance functioning and support treatment goals, utilizing a holistic and person-centered approach. Services are delivered in various settings, including homes, and may require evening and weekend availability.

Responsibilities

  • Provide individualized services based on strengths, needs, abilities, and preferences identified collaboratively with clients and families.
  • Engage parents in treatment to improve clinical outcomes for children.
  • Utilize a System of Care philosophy to provide services to children and families.
  • Support clients and families in completing goals identified on Wellness Plans and reassess needs as necessary.
  • Assist families in accessing and arranging services and resources to achieve their goals.
  • Maintain contact with clients during hospitalizations and participate in discharge planning.
  • Provide educational information and skill-building to clients and families regarding daily living skills and symptom management.
  • Develop and implement crisis intervention plans and provide crisis intervention services as needed.
  • Assist clients and families in developing natural support systems.
  • Coordinate with interagency team members to advocate for the mental health needs of clients and families.
  • Complete required documentation collaboratively with clients and families within the required timeframe.
  • Meet service provision expectations per client needs and agency guidelines.

Requirements

  • High School diploma or GED required; Associate's, Bachelor's, or Master's degree in a human service-related field preferred.
  • At least four years of experience in a relevant field if no degree is obtained.
  • At least two years of experience if an Associate's degree is the highest education obtained.
  • At least one year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • Experience working with individuals with psychiatric, substance use, and/or developmental disabilities is required.
  • A valid driver's license and agency-established minimum automobile coverage are required.
