The Community Support Specialist plays a crucial role in providing integrated behavioral health community support services to children and their families. This position is designed to engage both the child and the parent or caregiver in learning and practicing essential skills that will help them meet their goals and enhance their overall functioning. Services are delivered flexibly, accommodating the needs of clients during the day, evenings, and weekends, ensuring that support is available when it is most needed. The Community Support Specialist will work collaboratively within a team environment, contributing to a holistic approach that emphasizes the strengths and needs of each client. This role is vital in helping families navigate the complexities of mental health treatment and support, fostering a collaborative relationship that promotes positive outcomes. In this position, the Community Support Specialist will utilize a motivational, stage-matched approach to provide individualized services tailored to the unique strengths, needs, abilities, and preferences of each client and their family. This includes working closely with parents to ensure their active participation in the treatment process, which is essential for achieving improved clinical outcomes. The specialist will employ a System of Care philosophy, focusing on a person-centered, strengths-based perspective that incorporates coaching on the Eight Dimensions of Wellness. This comprehensive approach addresses various domains of wellness, including physical and mental health, to support the client and family in achieving their goals. The responsibilities also include maintaining ongoing contact with clients and families, particularly during hospitalization, to facilitate discharge planning and aftercare. The Community Support Specialist will provide educational information and skill-building opportunities in areas such as daily living skills, coping strategies, and symptom management. Additionally, they will assist families in developing natural support systems and coordinate with interagency team members to advocate for the mental health needs of the child and family. Documentation is a critical aspect of this role, requiring timely and accurate completion of all necessary records in accordance with agency policies. Overall, this position is integral to the mission of Compass Health Network, which is to inspire hope and promote wellness in the communities it serves.

Responsibilities

  • Provide individualized services based on strengths, needs, abilities, and preferences identified collaboratively with client/family.
  • Engage parents in treatment to improve clinical outcomes for the child.
  • Utilize a System of Care philosophy and a holistic, person-centered, strengths-based perspective in service delivery.
  • Support client/family in completing goals identified on Wellness Plans and reassess needs as necessary.
  • Assist families in accessing, locating, and arranging services and resources to achieve their goals.
  • Maintain contact with clients/families during hospitalization and participate in discharge planning.
  • Provide educational information and skill-building regarding daily living skills, coping skills, and symptom management.
  • Develop and implement crisis intervention plans and provide crisis intervention services as needed.
  • Assist client/family in developing natural support systems and coordinate with interagency team members.
  • Complete documentation collaboratively with client and family within required timeframes.

Requirements

  • High School diploma or GED required; Associate's, Bachelor's, or Master's degree in a human service-related field preferred.
  • At least four years of experience in a relevant field if no degree is obtained.
  • At least two years of experience required if an Associate's degree is the highest education obtained.
  • At least one year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • Experience working with individuals with psychiatric, substance use, and/or developmental disabilities is required.
  • A valid driver's license and agency-established minimum automobile coverage are required.
