About the position

The Community Support Specialist plays a crucial role in developing, implementing, and monitoring individualized habilitation plans for consumers. This position focuses on therapeutic rehabilitative skill development and the application of Illness Management and Recovery (IMR) principles. The specialist coordinates mental and physical health services, provides crisis intervention, and ensures active rehabilitation and community integration for consumers.

Responsibilities

• Contribute to the development and implementation of individualized habilitation plans.
• Monitor and update habilitation plans as needed.
• Implement therapeutic rehabilitative skill development.
• Apply Illness Management and Recovery (IMR) principles and interventions.
• Coordinate and manage mental health and physical health services.
• Provide crisis intervention when necessary.
• Maintain an appointment schedule and meet work productivity requirements, including 60% face-to-face contact weekly.
• Operate an Agency vehicle and maintain an acceptable driving record.

Requirements

• Bachelor's degree from an accredited college or university in a helping profession (e.g., social work, counseling, psychiatric rehabilitation, psychology).
• Minimum of 1 year of professional experience in a community-based behavioral health setting, or 2 years if the degree is in a non-helping profession.
• Certified Psychiatric Rehabilitation Practitioner (CPRP) may substitute for one year of experience.

Nice-to-haves
