About the position

The Community Support Specialist (CSS) plays a vital role in assisting wounded warriors with moderate-to-severe traumatic brain injuries, spinal cord injuries, or other neurological conditions. This part-time position focuses on helping clients regain independence and reintegrate into their communities by providing functional support, developing individualized care plans, and facilitating access to resources and activities. The CSS works collaboratively with clients to establish goals, provide advocacy, and support in various life skills, including household management, financial decision-making, and social engagement.

Responsibilities

  • Provide functional support to clients based on their Individualized Care Plan to improve independence.
  • Assist clients in reintegrating into their communities to encourage a healthy level of independence.
  • Advocate for clients and help them identify choices and make informed decisions regarding their activities and life.
  • Support household management by maintaining a clean and safe home environment and establishing efficient routines.
  • Train clients in financial management, including decision-making on purchases and meeting financial obligations.
  • Facilitate participation in recreational and community activities to promote socialization and peer relationships.
  • Help clients improve community living and integration skills in various settings such as shopping, church, and volunteer opportunities.
  • Provide training or assistance with mobility in the home and accessing public transportation.
  • Aid clients in developing effective communication and social skills.
  • Conduct exercises or reinforce therapeutic programs as defined by therapists.
  • Educate clients on problem-solving skills and strategies.
  • Accompany clients to community locations and appointments to help them access resources and services.
  • Serve as a mentor for clients and maintain communication with clients, families, caregivers, and referral sources.
  • Participate in team meetings and training to enhance job skills and personal growth.

Requirements

  • Experience with individuals with Traumatic Brain Injury is required.
  • Military background is a plus.
  • Must have a reliable computer for case management and daily documentation.
  • Experience with Microsoft Word, Excel, and Adobe Acrobat Reader is required.
  • Bachelor's degree in a related field or 1-2 years of experience working with adults who are developmentally disabled or related experience preferred.
  • Must maintain a valid driver's license and pass a background check.
  • Must have personal automobile insurance with minimum coverage of $100,000/$300,000/$50,000.

Nice-to-haves

  • Experience in social services, psychology, direct care, human services, advocacy, or as a life skills coach.
