About the position

The Community Support Specialist role at Simple Steps, LLC is a part-time position focused on providing high-quality home and community services to Veterans with neurological disorders, physical disabilities, and traumatic brain injuries. The specialist will work closely with Case Managers and family members to implement care plans and support clients in achieving their goals through various activities and training.

Responsibilities

• Support and carry out quality client care plans for Veterans.
• Educate clients and their families about health care plan services and procedures.
• Document client progress and ensure accurate invoicing.
• Engage in social, recreational, wellness, educational, and individualized life skills training activities.
• Travel to clients' homes and transport clients as needed.
• Communicate effectively with health care professionals, clients, and families.
• Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader.

Requirements

• High School Diploma or GED.
• Experience with physical and cognitive impairments/disabilities.
• Experience with Traumatic Brain Injury and Post-Traumatic Stress Disorder.
• Veteran or military experience is preferred.
• Valid Driver's License and reliable transportation with agency-required auto liability insurance.
• CPR/AED/First Aid Certification from American Red Cross or American Heart Association.
• Access to a private personal laptop or desktop computer for HIPAA compliance.

Nice-to-haves

• Experience in Microsoft Word, Excel, and Adobe Reader.
