About the position

The Community Support Specialist at Freeman Health System plays a crucial role in delivering community support services to individuals affected by various behavioral health issues, including substance use, trauma, and mental illness. This position emphasizes the use of trauma-informed and person-centered approaches, aiming to promote the highest levels of independence and autonomy for the individuals served. The specialist is expected to demonstrate effective time management and multitasking skills to independently manage an assigned caseload, ensuring that clients receive the necessary support tailored to their individual needs. The role requires the specialist to arrange their work schedule to see clients regularly, adhering to the unique requirements of each individual. This includes managing multiple deadlines to ensure that services are provided within established expectations. The Community Support Specialist will collaborate with clients to develop individualized person-centered plans, which are essential for guiding ongoing treatment services. These plans must be completed and reviewed within established timelines and in accordance with program requirements. In addition to developing treatment plans, the specialist will monitor and assess clients' participation and progress in treatment, ensuring adherence to the individualized plans. They will provide treatment interventions that are medically necessary and ethical, while also communicating relevant clinical information with supervisors and other key clinical staff regarding clients' critical care needs. Documentation of client safety planning and hospitalization interventions must be completed per established guidelines, and all required documentation must meet quality standards set by state and federal regulations, as well as CARF certification requirements. The Community Support Specialist is also responsible for meeting or exceeding billing and productivity expectations as established by the Program Director. Collaboration with coworkers and treatment team members is essential to support clients in achieving their recovery goals. The specialist will ensure that clients have access to medical insurance benefits and community resources based on their income eligibility, and they will perform all other duties as assigned.

Responsibilities

  • Collaborates with clients to develop individualized person-centered plans and reviews/updates to guide ongoing treatment services.
  • Effectively monitors and assesses clients' participation and progress in treatment according to and in adherence with individualized person-centered plans.
  • Provides treatment interventions which are medically necessary, ethical, and in adherence with clients' individualized person-centered plans.
  • Communicates all relevant clinical information with supervisor and other key clinical staff regarding clients' critical care needs in a timely manner.
  • Documents client safety planning and hospitalization interventions per established guidelines.
  • Ensures that all required documentation is completed within established timelines and according to quality standards set by state and federal regulations, CARF certification requirements, and program expectations.
  • Meets or exceeds billing and productivity expectations as established by the Program Director.
  • Supports coworkers and teammates in providing trauma-informed, person-centered services to clients.
  • Collaborates effectively with treatment team members to support clients in achieving their chosen recovery goals.
  • Completes and submits all required/requested training and HR compliance documents timely.
  • Ensures clients have applied, receive, and maintain medical insurance benefits as well receive available community resources based on client's income eligibility.
  • Performs all other duties as assigned.

Requirements

  • Experience in social services or a related field.
  • Knowledge of behavioral health issues, including substance use, trauma, and mental illness.
  • Strong communication skills to effectively collaborate with clients and team members.
  • Ability to manage a caseload independently and effectively prioritize tasks.
  • Familiarity with documentation standards and regulatory requirements in the healthcare field.

Nice-to-haves

  • Experience with trauma-informed care practices.
  • Certification in mental health first aid or similar training.
  • Bilingual skills to communicate with a diverse client population.
