About the position

The Community Support Specialist plays a vital role in providing services to consumers with serious mental illness who reside in 24-hour supervised, intermittent support residential settings, as well as in independent and community living settings. This position involves assisting with referrals for residential services, particularly for adults who have recently experienced crisis events, as well as for special targeted populations and community services that align with individualized treatment plans. As a Community Support Specialist, you will assess the overall functioning of consumers, identifying their problems, needs, skill strengths, and deficits. You will assume a lead role in the consumer-centered planning process, developing and implementing strategies that cater to their unique needs. Your responsibilities will include implementing selected aspects of the treatment plan, which may involve skill building, counseling, advocacy, and fostering connections with community, family, and natural supports. In this role, you will ensure that consumers have access to community service providers, entitlement programs, and housing alternatives. You will advocate for individuals' basic needs and support their development of natural resources and personal independence by teaching self-advocacy skills. This position requires a Bachelor's degree in the human services field and preferably one year of direct care experience working with persons with disabilities. Additionally, you must be able to achieve QIDP status to work with individuals with developmental disabilities and have MHP status to work with those experiencing mental illness. Reliable transportation, a valid driver's license, proof of insurance, and insurability are also required. Employment with Bridgeway is conditional upon the successful completion of all background checks.

Responsibilities

Requirements
