About the position

The Community Support Specialist plays a vital role in providing individual and group instruction and support to participants in both facility and community-based settings. This position is responsible for developing and implementing Individual Plans in collaboration with participants, their families, and support teams, ensuring that services are delivered effectively and in accordance with established goals. The specialist also facilitates community engagement and skill development for participants, helping them to achieve their personal objectives.

Responsibilities

  • Conduct individual and/or group instruction and service provision to a caseload of participants.
  • Develop and implement Individual Plans in collaboration with program participants, parents, and social workers.
  • Maintain supports and services for goals within the allotted budget.
  • Review plans quarterly to assess progress and identify barriers to goal achievement.
  • Create and maintain confidential files for participants.
  • Develop and maintain a network of community resources for implementing Individual Plans.
  • Create or coordinate learning materials and environments tailored to participant needs.
  • Provide support to program participants in the community.
  • Work with participants to increase motivation and provide consistent reinforcement of learning.
  • Facilitate excursions for participants that relate to new skills being developed through program services.

Requirements

  • Associates or Bachelor's degree (B.A.) preferred, with one year of related experience; or equivalent combination of education and experience including a high school diploma and 1-2 years of related experience.
  • Ability to read and interpret safety rules, operating instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups.
  • Mathematical skills to perform basic calculations and interpret data.
  • Common sense reasoning to carry out instructions and solve problems.

Nice-to-haves

  • Experience working with individuals with disabilities.
  • Knowledge of community resources and support services.
