About the position

The Community Support Specialist position at Queen of Peace Center is designed to provide essential support to women with substance-use disorders, their families, and at-risk youth. The role is centered around family-centered care, focusing on treatment, prevention, education, and housing. The specialist will work closely with clients to develop and implement individual rehabilitation plans aimed at maximizing their adjustment and functioning within the community. This includes helping clients achieve sobriety, sustain recovery, and promote independence and responsibility through the involvement of natural support systems. In this role, the Community Support Specialist will engage in various activities tailored to the specific needs of each client. This includes developing recovery goals, identifying strengths and barriers, and assisting in the creation of treatment plans. The specialist will also help clients restore skills and resources that may have been negatively impacted by substance use, and will participate in interdisciplinary team meetings to ensure a comprehensive approach to each individual's rehabilitation plan. The position requires active participation in the client's recovery journey, including monitoring service delivery, advocating for necessary services, and providing experiential training in life skills. The Community Support Specialist will also be responsible for maintaining communication with clients, particularly those who have missed appointments, to re-engage them in their recovery efforts. The role includes planning for discharge and ensuring that clients are equipped with the necessary tools and resources to continue their recovery journey independently. The position requires flexibility, as the schedule includes one weeknight per week and one weekend coverage per month, along with the ability to transport clients as needed.

Responsibilities

  • Develop recovery goals and identify needs, strengths, skills, resources, and supports for clients.
  • Assist in the development of treatment plans.
  • Help clients develop natural supports to achieve and maintain recovery.
  • Restore skills and resources negatively impacted by substance use.
  • Participate in interdisciplinary team meetings to identify strengths and needs related to rehabilitation plans.
  • Attend periodic meetings with team members and clients to review and update rehabilitation plans.
  • Contact clients who have missed appointments within 72 hours to promote recovery efforts.
  • Arrange and refer clients for services and advocate for quality services.
  • Monitor service delivery by external providers and ensure communication and coordination of services.
  • Locate and coordinate services and resources to resolve crises.
  • Provide experiential training in life skills and resource acquisition.
  • Provide information and education according to the rehabilitation plan.
  • Plan for client discharge.
  • Meet monthly billable expectations consistently.
  • Transport clients in the employee's vehicle.
  • Provide group education.
  • Assist clients in establishing treatment goals.
  • Schedule includes one weeknight per week and one weekend coverage per month.

Requirements

  • Bachelor's degree in social work, counseling, psychology, or closely related field; or a combination of higher education and qualifying experience equaling four (4) years; or an Associate of Applied Science in Behavioral Health Support degree from an approved institution; or four (4) years of qualifying experience.
  • Valid Class E driver's license, vehicle, and current automobile liability insurance.
  • Current and annual TB and CPR certification.
