About the position

The Community Support Specialist at Compass Health Network plays a crucial role in providing integrated, holistic, community-based support services to clients dealing with mental health and substance abuse issues. This position focuses on empowering clients through individualized and group education, assisting them in achieving their recovery and resiliency goals, and facilitating access to necessary public services.

Responsibilities

• Provide individualized and group education services based on clients' strengths, needs, abilities, and preferences.
• Assist clients in identifying their strengths and needs regarding substance abuse and mental illness, and participate in the development of Wellness Plans.
• Incorporate coaching surrounding the Eight Dimensions of Wellness into service provision.
• Support clients in completing goals identified on Wellness Plans and review these plans as needed.
• Aid clients in accessing public services such as financial assistance, housing, and food.
• Maintain contact with hospitalized clients and participate in discharge planning.
• Provide education and skill-building in daily living skills, employment, symptom management, and budgeting.
• Assist clients in developing personal support systems, including family and recovery communities.
• Accompany clients to appointments to address medical or health needs.
• Follow up with clients regarding appointments and completion of necessary forms.

Requirements

• Experience in providing community-based support services.
• Knowledge of mental health and substance abuse issues.
• Ability to develop and implement Wellness Plans collaboratively with clients.
• Strong communication and interpersonal skills.
• Experience in facilitating group education and skill-building sessions.

Nice-to-haves

• Experience working with diverse populations.
• Certification as a Qualified Mental Health Professional (QMHP).
• Familiarity with local public services and resources.
