The Community Support Specialist plays a crucial role in providing individual and group services to adults and youth, focusing on case management interventions within various settings including the office, home, and community. This position requires a compassionate and proactive approach to support individuals in navigating their mental health and physical health challenges. The specialist will act as a liaison between inpatient hospitalizations, emergency rooms, and nursing homes to ensure a smooth transition for clients during discharge and follow-up care. In this role, the specialist will provide person-centered case management, which includes individual and group counseling tailored to the unique needs of each client. Advocacy is a key component, as the specialist will work diligently to help individuals obtain necessary resources, housing, and healthcare services. The position also involves facilitating interventions that focus on skill development, enabling clients to manage their mental health and physical health issues effectively. The specialist will utilize evidence-based curricula to teach essential skills related to symptom management, recovery, interpersonal relationships, daily living, problem-solving, healthy living, social connectedness, medication management, and communication. Additionally, the role includes providing appropriate linkage and referrals to assist clients with entitlements, actively participating in agency meetings, trainings, and supervision, and completing necessary documentation of services. Crisis intervention may also be required on a rotating call basis, ensuring that support is available when needed.
