About the position

The Community Support Specialist plays a crucial role in the development, implementation, monitoring, and updating of individual habilitation plans for consumers. This position involves therapeutic rehabilitative skill development and the application of Illness Management and Recovery (IMR) principles and interventions. The specialist is responsible for coordinating and managing both mental health and physical health services, providing crisis intervention as needed. The role also includes the provision and coordination of services aimed at active rehabilitation and support, which are essential for restoring functioning and achieving community integration. The specialist must maintain their own appointment schedule and meet work productivity requirements, while also being open to other duties as assigned by management staff. Community Support Services is dedicated to assisting Burlington County adults with serious mental illnesses to live successfully within the community. The services offered include case management, housing assistance, supportive counseling, advocacy, service linkages, illness management, peer support, life skills training, education and employment assistance, transportation, and 24-hour on-call support. Additionally, Enhanced Community Support Services are available for adults transitioning from state psychiatric hospitals, providing a higher level of support and case management to facilitate their reintegration into the community.

Responsibilities

  • Contribute to the development, implementation, monitoring, and updating of consumers' individual habilitation plans.
  • Facilitate therapeutic rehabilitative skill development.
  • Implement Illness Management and Recovery (IMR) principles and interventions.
  • Coordinate and manage mental health and physical health services.
  • Provide crisis intervention as needed.

  • Maintain own appointment schedule and meet work productivity requirements.
  • Perform other duties as assigned by management staff.

Requirements

  • Bachelor's Degree from an accredited college or university in a helping profession (social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice).
  • If the degree is in a field other than a helping profession, a minimum of two years of professional experience in a community behavioral healthcare setting is required.
  • Certification as a psychiatric rehabilitation practitioner may substitute for one year of experience.
