About the position

The Community Support Specialist plays a crucial role in providing home-based case management services to families in need. This position focuses on ensuring quality care and support for clients, particularly children and youth, to help them live their best lives. The specialist will engage in various responsibilities, including parenting education, family assessments, and facilitating visitations, while maintaining thorough documentation and participating in regular supervision meetings.

Responsibilities

• Provide home base case management services
• Provide Parenting Education
• Understand and implement evidenced based moralities
• Complete Casework Plan, weekly case notes, and monthly progress reports
• Attend Supervision/Staffing bi-weekly
• Facilitate and supervise family visitations
• Assess family functioning and parent skills
• Other duties as assigned

Requirements

• Bachelor's degree in social work, psychology, marriage and family therapy, or related human service field required
• Previous experience preferred
• Valid Driver's License in good standing and reliable transportation & auto insurance that complies with state law
• Successful clearance of local, state, and national background checks
