About the position

The Community Support Specialist at Compass Health Network plays a crucial role in providing integrated, holistic, community-based support services to clients facing mental health and substance abuse challenges. This position focuses on empowering clients through individualized education and support, helping them achieve their recovery and resiliency goals while navigating various public services and resources.

Responsibilities

• Provide individualized and group education services based on clients' strengths, needs, abilities, and preferences.
• Assist clients in identifying their strengths and needs regarding substance abuse and mental illness, and participate in the development of Wellness Plans.
• Incorporate coaching surrounding the Eight Dimensions of Wellness into service provision.
• Support clients in achieving goals identified on Wellness Plans and update these plans as necessary.
• Aid clients in accessing public services such as financial assistance, housing, and food resources.
• Maintain contact with hospitalized clients and participate in discharge planning and aftercare development.
• Provide education and skill-building in daily living skills, employment, symptom management, and budgeting.
• Assist clients in developing personal support systems, including family and recovery communities.
• Accompany clients to appointments to address medical or health needs and follow up on their progress.

Requirements

• Bachelor's degree in a relevant field or equivalent experience.
• Experience in providing community-based support services.
• Knowledge of mental health and substance abuse issues.
• Strong communication and interpersonal skills.
• Ability to work collaboratively with clients and their families.

Nice-to-haves

• Qualified Mental Health Professional (QMHP) certification.
• Experience in motivational interviewing techniques.
• Familiarity with the Eight Dimensions of Wellness framework.
