About the position

The Community Support Specialist plays a crucial role in assisting individuals with intellectual and developmental disabilities to live independently. This position focuses on enhancing self-direction, independent living skills, and community integration while providing support in various aspects of daily living. The role requires flexibility in hours based on individual customer needs and includes participation in an on-call crisis intervention rotation.

Responsibilities

  • Assesses and monitors a client's acclimation to community living.
  • Monitors client participation and progress in various programs to ensure services align with the individual support plan (ISP).
  • Participates in the development or revision of the ISP and communicates with the case manager regarding potential goals and objectives.
  • Provides individual assistance to customers in accessing needed health care and mental health services, including accompanying clients to appointments.
  • Conducts ongoing monitoring of customer's risks, strengths, and weaknesses through interviews and reviews of customer records.
  • Encourages medication compliance and provides education regarding medications and monitoring of side effects.
  • Assists customers in accessing public services, including financial and medical assistance, and helps meet immediate needs for food, shelter, and clothing.
  • Intercedes on behalf of clients within the community to help them achieve and maintain community living standards.
  • Trains, coaches, and supports customers and staff in daily living skills, budgeting, and maintaining an independent residence.
  • Assists in creating personal support systems involving family members and significant others.
  • Encourages and promotes recovery efforts, customer independence, and self-care responsibilities.
  • Notifies the Program Director of any changes in customer needs.

Requirements

  • Active RN License in good standing or a Bachelor's Degree with at least 1 year of experience or an Associate Degree with at least 3 years of experience in a related field.
  • Experience in crisis intervention and caregiving.
  • Mid-level organizational management skills.

Nice-to-haves

  • Experience in community integration and social skills training.
  • Knowledge of public services and resources available for individuals with disabilities.
