About the position

The Community Support Specialist plays a crucial role in providing integrated, holistic, community-based support services to individuals facing mental health challenges. This position is essential in addressing the critical service needs of the communities served by Compass Health Network. The specialist employs a motivational, stage-matched approach to deliver individualized and group education services tailored to the strengths, needs, abilities, and preferences of clients and their families. The focus is on supporting clients in their journey toward recovery and resiliency goals, ensuring that services are delivered from a holistic perspective that incorporates the Eight Dimensions of Wellness. In this role, the specialist assists clients in identifying their strengths and needs related to substance abuse and mental illness, participating in the development of personalized Wellness Plans. They provide ongoing support to clients in achieving their goals, regularly reviewing and updating these plans as necessary. The specialist also aids clients in accessing public services, such as financial assistance, housing, and food resources, and maintains contact with clients who are hospitalized to facilitate discharge planning and aftercare. Additionally, the Community Support Specialist is responsible for educating clients in essential life skills, including daily living skills, employment readiness, symptom management, and budgeting. They work collaboratively with clients to develop personal support systems and provide individual assistance in accessing necessary services, including accompanying clients to appointments. The specialist is also tasked with re-engaging clients who have missed appointments and ensuring that service provision meets both client needs and company guidelines. The role requires active communication with an integrated health team, including psychiatrists, primary care physicians, and other specialists, to ensure comprehensive care for clients. The specialist serves as a liaison between clients and various stakeholders involved in their treatment, including family members, employers, and legal entities. Cultural competence is emphasized in service delivery to meet the unique needs of each client and their families. Driving is an essential function of this position, as transportation assistance may be required for clients.

Responsibilities

  • Provide individualized and group education services based on clients' strengths, needs, abilities, and preferences.
  • Assist clients in identifying their strengths and needs regarding substance abuse and mental illness.
  • Participate in the development of Wellness Plans with clients.
  • Support clients in achieving goals identified on Wellness Plans and review them as needed.
  • Aid clients in accessing public services such as financial, housing, and emergency assistance.
  • Maintain contact with hospitalized clients and participate in discharge planning.
  • Provide education and skill-building in daily living skills, employment, symptom management, and budgeting.
  • Assist clients in developing personal support systems, including family and recovery communities.
  • Provide individual assistance to clients in accessing needed services and accompany them to appointments.
  • Follow up with clients regarding appointments and necessary documentation.
  • Re-engage clients who have unexcused absences from the CSTAR program.
  • Meet service provision expectations per client needs and company guidelines.
  • Complete documentation and paperwork within required timeframes.
  • Communicate actively with integrated health team members regarding client wellness.
  • Seek input from integrated health team providers on clients' Wellness Plans.
  • Make referrals to additional services/providers as needed for clients' integrated health needs.
  • Function as a liaison to others involved in clients' treatment, including family, employers, and legal entities.
  • Seek culturally competent services to meet the unique needs of clients and families.
  • Assist clients with transportation as clinically indicated.

Requirements

  • High School/GED required.
  • Associate's/Bachelor's/Master's degree in a human service-related field preferred.
  • At least four (4) years of experience required if no degree is obtained.
  • At least two (2) years of experience required if an Associate's degree is the highest education obtained.
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves
