About the position

The Community Support Specialist position at Simple Steps, LLC is a part-time role focused on enhancing the quality of life for Veterans with neurological disorders, physical disabilities, and traumatic brain injuries. This position is designed to provide high-quality home and community services, ensuring that Veterans receive the support they need to thrive. The Community Support Specialist will work closely with Case Managers and family members to deliver individualized care plans that address the unique needs of each client. This role requires a compassionate individual who can effectively communicate with clients, families, and healthcare professionals, ensuring that all parties are informed and engaged in the care process. In this role, the Community Support Specialist will be responsible for implementing patient care plans through various activities that promote social interaction, wellness, and life skills development. The position involves traveling to clients' homes, providing transportation as needed, and engaging clients in community activities. The Specialist will also be responsible for maintaining accurate documentation of client progress and SMART goals, utilizing tools such as Microsoft Word, Excel, and Adobe Reader to track and report on these metrics. The ability to work independently and manage time effectively is crucial, as the Specialist will often be required to operate autonomously while ensuring that client needs are met. The position offers flexible scheduling, with the potential for increased hours based on the needs of Veterans. Mileage and expense reimbursement are provided, making this an attractive opportunity for individuals looking to make a meaningful impact in the lives of those who have served our country. Candidates should be prepared to lift up to 50 lbs and engage in various physical activities as part of their role.

Responsibilities

  • Support and implement quality client care plans for Veterans.
  • Engage in dynamic social, recreational, wellness, educational, and individualized life skills training and activities.
  • Communicate effectively with healthcare professionals, clients, families, and the general public.
  • Document client progress and SMART goals using Microsoft Word, Excel, and Adobe Reader.
  • Transport clients during visits and assist with community activities and appointments.
  • Maintain accurate and complete documentation for invoicing purposes.
  • Work independently and manage time effectively to meet client needs.

Requirements

  • High School Diploma or equivalent.
  • Experience with physical and cognitive impairments/disabilities.
  • Experience with Traumatic Brain Injury and Post-Traumatic Stress Disorder.
  • Veteran or military experience preferred.
  • CPR/AED/First Aid Certification from American Red Cross or American Heart Association.
  • Valid driver's license and agency-required auto liability insurance (100k/300k Bodily Injury).
  • Access to a private personal laptop or desktop computer for HIPAA compliance.

Nice-to-haves

  • Experience working with Veterans or military personnel.
  • Knowledge of community resources available for individuals with disabilities.
  • Strong organizational skills and ability to assist with household management.
