About the position

The Community Support Specialist at Mercedes Diaz Homes Inc. is responsible for providing support to individuals with intellectual and developmental disabilities in their homes and communities. This role involves coordinating services, conducting intake assessments, and advocating for individuals in various settings. The position emphasizes the importance of compassion, communication, and community engagement, aiming to enhance the quality of life for program participants and their families.

Responsibilities

• Coordinate services for individuals with developmental disabilities.
• Conduct intake assessments and interviews with prospective individuals and their families.
• Maintain open communication with stakeholders including individuals, parents, and agencies.
• Complete daily case notes and quarterly progress reports.
• Advocate for individuals in court proceedings as part of the Judicial Intervention and Diversion Compliance Program.
• Build community partnerships to enhance resources for individuals and their families.
• Participate in required training and team meetings to develop skills and knowledge.

Requirements

• High school diploma or GED equivalent.
• Bachelor's degree in psychology, behavioral science, social services, or related discipline, or four years of relevant experience.
• Current driver's license and California automobile insurance coverage.
• Ability to read, write, and speak English; bilingual in English/Spanish preferred.
• Experience working with individuals with intellectual or developmental disabilities.
