About the position

Community Solutions (CS) has been a cornerstone in the Santa Clara County community since its inception as a teen drop-in center in 1972. Over the years, CS has expanded its services to address the evolving needs of the community, now providing a wide range of prevention, intervention, treatment, and residential services to over 6,400 clients annually. The organization focuses on helping individuals overcome significant challenges, including mental health issues, substance abuse, trauma, severe family dysfunction, sexual and domestic violence, and human trafficking. As a Community Support Specialist, you will work under the general supervision of the Program Manager or their designee. Your primary responsibility will be to assist clients and their families by delivering essential services as mandated by various contracts. This role requires you to conduct screenings, intake assessments, and develop individualized Transformational Care Plans (TCP) for clients. You will also be responsible for maintaining detailed documentation of client progress and implementing appropriate resources and referrals to professional services without delay. In addition to providing rehabilitation counseling and crisis intervention, you will teach life skills and assist clients in accessing community services. Your role will involve collaborating with psychiatrists to monitor medication support services and ensuring that all program documentation complies with agency and regulatory requirements. You will also be expected to maintain excellent customer service skills and develop collaborative relationships with community organizations and system providers. Flexibility in your schedule is essential, as you may need to work evenings and weekends as required. Responsibilities

  • Conduct screening, intake, and assessments as required.
  • Assess clients' needs and goals to determine appropriate resources, including referrals to professional services.
  • Implement individual and family Transformational Care Plans (TCP) and maintain detailed documentation of progress.
  • Provide rehabilitation counseling, advocacy, crisis intervention, and parenting education.
  • Support client developmental growth and monitor medication support services in collaboration with psychiatrists.
  • Complete program documents in accordance with agency and regulatory requirements.
  • Coordinate services with advocacy programs, the legal system, and community-based providers.
  • Teach life skills and assist clients with educational and support activities.
  • Maintain excellent customer service skills with internal and external customers.
  • Develop and maintain collaborative relationships with community organizations and system providers.
  • Respond to phone calls and emails within 24 hours on business days.
  • Participate in agency meetings and contribute to quality improvement activities.
  • Complete all training requirements for compliance with agency and contract standards.
  • Participate in on-call and after-hours rotation as assigned.

Requirements

  • High School diploma or GED.
  • Four (4) years of direct service work experience in a related field.
  • Knowledge of empowerment, advocacy, and support techniques for victims/survivors of violence, mental illness, or substance abuse.

Nice-to-haves

  • Experience with Transformational Care Plans (TCP).
  • Familiarity with evidence-based practices and the wellness & recovery model.
