About the position

The Community Support Specialist plays a crucial role in providing support to consumers with serious mental illness in both residential and independent living settings. This position is designed to assist individuals who require 24-hour supervised care and those transitioning to community living. The specialist will work closely with consumers to develop individualized treatment plans that address their unique needs and challenges. This involves assessing the consumer's overall functioning, identifying problems, needs, and skill strengths, and leading the consumer-centered planning process. The specialist will implement various strategies, including skill building, counseling, and advocacy, to help consumers achieve their goals and improve their quality of life. In addition to direct support, the Community Support Specialist will ensure that consumers have access to necessary community services, entitlement programs, and housing alternatives. Advocacy is a key component of this role, as the specialist will work to promote the individual's basic needs and foster personal independence by teaching self-advocacy skills. The position requires a compassionate and dedicated individual who is committed to making a positive impact in the lives of those they serve. The specialist will also collaborate with community service providers and other stakeholders to create a supportive network for consumers, ensuring they receive comprehensive care and resources. This role is essential in bridging the gap between consumers and the services they need, ultimately empowering individuals to lead fulfilling lives within their communities.

Responsibilities

  • Assess consumer's overall functioning, identifying problems, needs, skill strengths and deficits.
  • Lead the consumer-centered planning process and develop implementation strategies.
  • Implement selected aspects of the treatment plan including skill building, counseling, and advocacy.
  • Ensure linkages to community service providers, entitlement programs, and housing alternatives.
  • Advocate for individuals' basic needs and promote personal independence through teaching self-advocacy skills.

Requirements

  • Bachelor's degree in the human services field required.
  • Prefer one year of direct care experience working with persons with disabilities.
  • Must be able to achieve QIDP status to work with persons with developmental disabilities.
  • Must have MHP status to work with persons with mental illness.
  • Must have reliable transportation, valid driver's license, proof of insurance, and be insurable.
