About the position

The Community Support Specialist at Family Ties, Inc. plays a crucial role in providing in-home and school-based assessment, counseling, and community integration services for families and children. The position focuses on skill development and resource assistance to help families maintain children safely in their homes, emphasizing the belief that families can overcome challenges with the right support and education.

Responsibilities

• Conduct skill-building activities with children/adolescents in alignment with treatment plans.
• Link parents/guardians with community resources and provide case management services.
• Teach and reinforce positive behavioral changes with parents/guardians.
• Act as a liaison between families and mental health professionals.
• Advocate for children/families, including writing reports and attending court proceedings.
• Develop Individual and Family Safety Plans in collaboration with families.
• Document all interactions and services provided to children/families.
• Create and submit clear documentation of services rendered.

Requirements

• Bachelor's degree in social work, psychology, or a related human services field.
• Two or more years of experience as a Community Support Individual, Paraprofessional, or Case Manager.
• Documented experience working with at-risk children, adolescents, and families.
• Excellent verbal and written communication skills.
• Demonstrated knowledge of and sensitivity to diverse cultures.
• Flexible work schedule, including evenings and weekends.
• Valid Georgia driver's license and ability to travel off-site.

Nice-to-haves

• Bilingual (fluent in Spanish and English) is a plus.
