About the position

As a Community Support Specialist at Keystone House Inc., you will play a vital role in providing case management services to agency clients. Your primary responsibilities will include assisting clients in accessing various entitlements, transportation, clinical services, vocational training, social support, and educational programs. This position requires a hands-on approach to skill teaching, ensuring that the training needs of clients are effectively met. You will also assist clients with essential daily activities such as menu planning, cooking, cleaning, personal care, and shopping, fostering their independence and self-sufficiency. In addition to direct client support, you will facilitate group sessions, documenting each client's progress on their goal activities in accordance with agency and regulatory standards. Your role will also involve assisting your supervisor in maintaining the facility, ensuring safety, security, and cleanliness in compliance with Department of Health regulations. A valid driver's license is required, as you may need to provide transportation for clients using your own vehicle when necessary.

Responsibilities

  • Provide case management services to agency clients.

Requirements

  • Experience in case management or a related field.
  • Ability to perform hands-on skill teaching.
  • Strong communication and interpersonal skills.
  • Valid driver's license and reliable transportation.
