About the position

ReDiscover is seeking a Community Support Specialist to join our Kansas City Assessment and Triage Center Department. For over 50 years, ReDiscover has been a non-profit community mental health center located in southeastern Jackson County, Missouri, dedicated to providing efficient and cost-effective treatment to individuals affected by mental illness or substance use. Our mission is to support our community, particularly those at risk, through a diverse team of over 700 passionate employees. The Kansas City Assessment and Triage Center (KC-ATC) is a comprehensive crisis and stabilization program designed to meet the needs of Kansas City's at-risk population. Our multidisciplinary staff serves as a vital resource for law enforcement, hospital emergency departments, and community mental health programs throughout the greater Kansas City area. As a Community Support Specialist, you will play a crucial role in maintaining and interacting with a caseload of clients, helping them achieve long-range outcomes related to symptom management, employment, and independent living based on their individual needs. Your responsibilities will include engaging with difficult-to-serve clients, investigating potential resources to facilitate access, and helping to remove barriers that may hinder their participation in community or social service programs. You will also be responsible for collecting and maintaining client information, documenting interactions within ReDiscover's designated electronic medical record, and collaborating with an internal multi-disciplinary team to ensure effective communication and coordination of client care. Responsibilities

  • Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
  • Facilitate each client's achievement of long-range outcomes of symptom management, employment, and independent living based on their needs.
  • Engage difficult-to-serve clients by investigating all potential resources to gain access and make connections.
  • Help remove barriers for clients, including assisting with participation in community or social service programs, referring clients to educational or working programs, and assisting with transportation barriers as identified in their treatment plan.
  • Collect and maintain information about clients and document interactions within ReDiscover's designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements

  • Availability to work a full-time schedule Monday-Friday.
  • 4-year degree in a social science field or 4 years of equivalent experience in lieu of degree.
  • A valid automobile driver's license, appropriate automobile insurance, and a safe, reliable, registered automobile available for use during work.
  • At least two (2) years clinical experience in direct service provision to children and adolescents in a mental health setting preferred.
  • Knowledge of community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral, and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word, Excel, and GSuite.

Nice-to-haves

  • Knowledge of community resources and health and social service systems in the Kansas City metro area.
  • Experience in establishing working relationships with community partners.
