About the position

Lumibility is a person-centered agency dedicated to serving individuals with intellectual and/or developmental delays. Established in 1957, we have been a leading provider of human services across various towns in Connecticut. The Community Support Specialist plays a crucial role in our mission by providing one-on-one services to adults with disabilities, focusing on building the skills necessary for maintaining independence in the community with minimal support. This position is full-time, requiring a commitment of 40 hours per week, typically scheduled between 8 AM and 6 PM, Monday through Friday. The role involves driving to various locations, including Madison, to provide essential support to our clients. As a Community Support Specialist, you will be responsible for supervising, guiding, and instructing individuals in community-based services. This includes facilitating volunteer opportunities and providing in-home services. Your support will encompass direct supervision, teaching independent living tasks, assisting with daily personal care, implementing behavioral interventions, and attending team meetings. You will also be tasked with developing and implementing person-centered activities that promote positive outcomes for the individuals you serve. In addition to direct support, you will maintain comprehensive medical, social, and programmatic files, documenting each person's progress and other required information, such as service goals and attendance records. Building and maintaining positive relationships with service participants, their families, co-workers, and community representatives is essential to this role. Your ability to provide transportation for service participants in accordance with Lumibility's safe-driving policies will also be a key responsibility.

Responsibilities

  • Provide supervision, guidance, and instruction to individuals served by Lumibility in assigned community-based services.
  • Develop and implement activities that are person-centered and promote positive outcomes.
  • Provide transportation for assigned service participants in accordance with Lumibility safe-driving policies and practices.
  • Maintain medical, social, and programmatic files by recording each person's progress and documenting required information.
  • Develop and maintain positive relationships with service participants, families, co-workers, and other community representatives.

Requirements

  • High school diploma or equivalent required; Bachelor degree in related field preferred.
  • Knowledge of learning and behavioral processes helpful.
  • Must possess a valid Connecticut driver's license with an acceptable driving history.
  • Ability to work flexible hours and travel to varying locations as needed.

Nice-to-haves

  • Experience working with individuals with disabilities.
  • Certification in CPR and First Aid.
