About the position

The Community Support Specialist at LifePath is a vital role within an interdisciplinary team dedicated to supporting older adults and individuals with disabilities. This position focuses on promoting health and well-being through education, coordination of services, and conducting member-centered assessments. The specialist assists with care transitions, follow-ups, and resource access, ensuring that individuals thrive in their home settings.

Responsibilities

  • Conduct case management assessments of referred individuals in home settings and carry out an individualized service plan.
  • Provide administrative and member onboarding support to interdisciplinary team members.
  • Provide skills training and assistance in areas of daily living activities and social skills.
  • Assist in developing communication and conflict resolution skills and act as a role model for interpersonal relationships.
  • Assist with individual and family advocacy needs including identifying and obtaining entitlements that meet member's needs.
  • Make appropriate referrals to other agencies as needed.
  • Serve as an advocate for members to obtain other needed services.
  • Maintain up-to-date knowledge of information about services available.
  • Assist participants with identifying appropriate behavioral health and medical services and accessing services of choice.
  • Facilitate utilization of professional providers who can assist with care plan goals.
  • Collaborate with and coordinate providers to ensure adequate implementation of service plans.
  • Provide support to members and caregivers to maintain placement stability and assist during periods of transition.
  • Assist in developing and maintaining a 'natural support network' of family, friends, and significant others in the participant's community.
  • Actively participate with the interdisciplinary team and provide consistent communication with members, support networks, and community partners involved in member's care plans.
  • Attend meetings as required.
  • Maintain accurate and timely documentation in accordance with agency policies and procedures.
  • Maintain statistics and reporting as required by the program.

Requirements

  • High school diploma or GED required; Bachelor's Degree preferred.
  • 1 year of relevant experience required.
  • Demonstrated experience working with older adults or individuals with disabilities.
  • Experience navigating benefits, resources, behavioral health systems, and long-term supports within the community.
  • Ability to foster self-sufficiency for individuals and families using problem-solving and practical skills.
  • Good listening skills and excellent oral and written communication skills.
  • Computer proficiency.
  • Ability to work independently and collaboratively to follow through with projects and meet deadlines.
  • Experience in advocacy, case management, supportive counseling, and goal-oriented problem solving.
  • Reliable transportation and willingness to travel throughout specified counties.
