About the position

The Community Support Specialist at Northpointe Behavioral Healthcare System plays a crucial role in supporting individuals with serious mental illness, emotional disturbances, substance abuse disorders, and intellectual/developmental disabilities. The primary purpose of this position is to coach, prompt, and train individuals to develop the skills necessary for independent living, achieve personal goals, and foster community inclusion.

Responsibilities

  • Assist clinical staff in administering and documenting therapeutic activities as trained per the individual plan of service.
  • Safely transport and accompany individuals to and from community needs according to the individual plan of service, ensuring proper loading and unloading procedures and seat belt use.
  • Attend and participate in staff/team meetings.
  • Advocate for and ensure that Recipient Rights are respected and protected.
  • Observe and document individuals' behaviors to facilitate assessment and development of treatment goals.
  • Provide verbal, nonverbal, behavioral, or physical interventions according to Northpointe training.
  • Interact with individuals and involve them in activities that are part of their daily routine, adapting as necessary for full participation.
  • Provide prompts and cueing with bathing, personal hygiene, and dressing according to the plan of service.
  • Record data into the individuals' electronic medical record.
  • Maintain a safe and therapeutic environment for individuals, coworkers, and the community.
  • Provide prompts and cueing with housekeeping and/or routine maintenance to maintain a safe and clean environment.
  • Maintain a safe driving record.

Requirements

  • High school diploma or general education degree (GED).
  • Experience working with individuals with developmental and/or intellectual disabilities preferred.
  • Experience working in a caregiving role preferred.
  • Valid driver's license preferred.
  • Certification as a nursing assistant preferred (re-certification required for compensation).
