About the position

The Community Support Specialist plays a vital role in providing family-centered care for women with substance-use disorders, their families, and at-risk youth. This position focuses on developing and implementing individual rehabilitation plans to help clients achieve sobriety, sustain recovery, and promote independence within the community. The specialist will work closely with clients to identify their needs, strengths, and resources, while also facilitating access to necessary services and supports.

Responsibilities

  • Develop recovery goals and identify needs, strengths, skills, resources, and supports for clients.
  • Assist in the development of treatment plans.
  • Help clients develop natural supports to achieve and maintain recovery.
  • Restore skills and resources negatively impacted by substance use.
  • Participate in interdisciplinary team meetings to identify strengths and needs for rehabilitation plans.
  • Attend periodic meetings with team members and clients to review and update rehabilitation plans.
  • Contact clients who have missed appointments to re-engage them in recovery efforts.
  • Arrange and refer clients for services and advocate for their needs.
  • Monitor service delivery by external providers and ensure communication and coordination of services.
  • Locate and coordinate services to resolve crises.
  • Provide experiential training in life skills and resource acquisition.
  • Educate clients according to their rehabilitation plans.
  • Plan for client discharge.
  • Meet monthly billable expectations consistently.
  • Transport clients in the employee's vehicle.
  • Provide group education sessions.
  • Assist clients in establishing treatment goals.
  • Schedule includes one weeknight per week and one weekend coverage per month.
  • Perform other duties as assigned.

Requirements

  • Bachelor's degree in social work, counseling, psychology, or a closely related field; or a combination of higher education and qualifying experience equaling four years; or an Associate of Applied Science in Behavioral Health Support degree from an approved institution; or four years of qualifying experience.
  • Valid Class E driver's license, vehicle, and current automobile liability insurance.
  • Current and annual TB and CPR certifications.
