About the position

The Community Support Specialist at Keystone House Inc. is responsible for providing case management services to clients with severe and persistent mental illness. This role involves assisting clients in accessing various programs and services, facilitating skill development, and ensuring a safe and clean environment in compliance with health regulations.

Responsibilities

• Provide case management services to agency clients.
• Assist clients with accessing entitlements, transportation, clinical, vocational, social, and educational programs.
• Perform direct skill teaching in a hands-on manner to meet client training needs.
• Assist clients with menu planning, cooking, cleaning, personal care, and shopping.
• Facilitate groups for client engagement and support.
• Document client's progress on goal activities per agency/regulatory standards.
• Assist supervisor in maintaining facility safety, security, and cleanliness in compliance with Department of Health regulations.
• Provide transportation in own vehicle when necessary.

Requirements

• Valid driver's license is required.
• Experience in case management or related field is preferred.

Nice-to-haves

• Experience working with individuals with mental illness.
• Skills in group facilitation and direct skill teaching.
