About the position

The Community Outreach Case Worker position is designed to assist clients in linking them to essential community resources such as housing and food assistance. This role involves working directly in the community, meeting clients face-to-face, and providing support to individuals dealing with severe mental health challenges. The position requires local travel and the use of a personal vehicle, with mileage reimbursement provided. The role operates on a Monday to Friday schedule, with no on-call, nights, weekends, or holidays required.

Responsibilities

  • Provides community support services to complex clients, utilizing treatment plan interventions for positive outcomes.
  • Completes timely documentation of services, detailing the need for the service, interventions provided, and individual responses.
  • Contacts individuals or referral sources following missed appointments to re-engage clients and promote recovery efforts.
  • Supports individuals in crisis situations.
  • Provides mentorship and job shadowing to community support colleagues.

Requirements

  • Bachelor's Degree in Human Services or a related field.
  • Valid Class D (IL) or Class E (MO) driver's license.

  • Related work experience with individuals suffering from mental health challenges.
