About the position

The Community Support Specialist position at Simple Steps, LLC is a part-time role based in Tucson, AZ, offering up to 10 hours of work per week, with the potential for increased hours based on the needs of Veterans. The primary responsibility of the Community Support Specialist is to enhance the quality of life for Veterans who are dealing with neurological disorders, physical disabilities, and traumatic brain injuries. This role involves delivering high-quality home and community services tailored to the unique needs of each client. Community Support Specialists are tasked with implementing and supporting client care plans, ensuring that resources are utilized effectively to meet the needs of clients. This includes educating clients and their families about healthcare services and procedures, maintaining accurate documentation, and providing timely data for invoicing purposes. The position requires collaboration with Case Managers and family members to deliver dynamic social, recreational, wellness, educational, and individualized life skills training and activities both in the home and community settings. The role demands a proactive approach to client engagement, requiring the specialist to travel to clients' homes, transport clients as needed, and communicate effectively with healthcare professionals, clients, and families. The position also emphasizes the importance of maintaining HIPAA compliance through the use of personal technology for documentation and communication. Overall, the Community Support Specialist plays a crucial role in supporting Veterans and ensuring they receive the care and assistance they need to thrive in their daily lives.

Responsibilities

  • Support and implement quality client care plans for Veterans.
  • Educate clients and their families about healthcare services and procedures.
  • Maintain accurate and complete documentation for client care.
  • Provide timely data for generating accurate invoices.
  • Engage in dynamic social, recreational, wellness, educational, and individualized life skills training and activities.
  • Travel to current or prospective clients' homes as needed.
  • Transport clients during visits when required.
  • Communicate effectively with healthcare professionals, clients, and families through verbal and written communication.
  • Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader.
  • Work independently and manage one's own time and the time of clients.

Requirements

  • High School Diploma or equivalent education.
  • Experience with physical and cognitive impairments/disabilities.
  • Experience with Traumatic Brain Injury and Post-Traumatic Stress Disorder.
  • Veteran or military experience is preferred.
  • Valid Driver's License and reliable transportation.
  • Agency-required auto liability insurance (100k/300k Bodily Injury).
  • CPR/AED/First Aid Certification from American Red Cross or American Heart Association.
  • Access to a private personal laptop or desktop computer for HIPAA compliance.

Nice-to-haves

  • Experience in community service or support roles.
  • Familiarity with Microsoft Office Suite (Word, Excel, Adobe Reader).
  • Ability to lift 50 lbs.
