About the position

The Community Support Specialist (CSS) plays a crucial role in providing integrated, holistic community support services to adults facing behavioral health and substance use disorders. This position is designed to assist individuals in their recovery and rehabilitation journey by working collaboratively with clients in various settings, including their homes, community spaces, and the office. The CSS employs a motivational, stage-matched approach to deliver individualized services tailored to the strengths, needs, abilities, and preferences of each client. This collaborative process involves the development of a Wellness Plan that focuses on the client's movement toward recovery and resiliency goals. In this role, the CSS will support clients in identifying their strengths and needs related to mental health and substance use, ensuring that services are provided from a holistic perspective. This includes coaching clients on the Eight Dimensions of Wellness, which encompasses physical and mental health. The CSS will assist clients in achieving the goals outlined in their Wellness Plans, regularly reviewing and updating these plans as necessary. Additionally, the CSS will help clients access public services such as financial assistance, housing, and food resources, while maintaining contact with clients who are hospitalized and participating in discharge planning. The CSS will also provide education and skill-building in essential areas such as daily living skills, employment, symptom management, and budgeting, with the aim of fostering independence. Building personal support systems for clients is another critical aspect of this role, which may involve collaboration with family members, legal guardians, and recovery communities. The CSS will ensure that clients receive the necessary mental health services, including accompanying them to appointments and following up on their progress. Collaboration is key in this position, as the CSS will actively communicate with other members of the integrated health team, including psychiatrists, primary care physicians, and specialists. This teamwork is essential for addressing all aspects of client wellness, including medication adherence and side effects. The CSS will also serve as a liaison to other parties involved in the client's treatment, such as family, employers, and legal entities. Cultural competence is emphasized, ensuring that services meet the unique needs of each client and their families. Driving is an essential function of this position, as the CSS may need to assist clients with transportation as clinically indicated.

Responsibilities

  • Provide individualized community support services to adults with behavioral health and substance use disorders.
  • Assist clients in identifying their strengths, needs, abilities, and preferences related to mental illness and substance use.
  • Develop and review Wellness Plans with clients, updating goals and objectives as needed.
  • Support clients in accessing public services such as financial assistance, housing, and food resources.
  • Maintain contact with hospitalized clients and participate in discharge planning.
  • Provide education and skill-building in daily living skills, employment, symptom management, and budgeting.
  • Help clients develop personal support systems, including collaboration with family and recovery communities.
  • Assist clients in accessing mental health services and accompany them to appointments as needed.
  • Follow up with clients regarding appointments and necessary documentation.
  • Communicate actively with integrated health team members regarding client wellness and treatment plans.

Requirements

  • High School diploma or GED required; Associate's, Bachelor's, or Master's degree in a human service-related field preferred.
  • At least four years of experience working with individuals with psychiatric, substance use, and/or developmental disabilities if no degree is obtained.
  • At least two years of experience required if an Associate's degree is the highest education obtained.
  • At least one year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • A valid driver's license and agency-established minimum automobile coverage are required.
