About the position

The Community Support Specialist role at Simple Steps, LLC focuses on enhancing the quality of life for Veterans with neurological disorders, physical disabilities, and traumatic brain injuries. This part-time position involves providing high-quality home and community services, supporting client care plans, and engaging with clients and their families to ensure effective communication and documentation. The role requires collaboration with Case Managers and family members to deliver individualized support through various activities.

Responsibilities

• Support and implement quality client care plans for Veterans.
• Educate clients and their families about health care services and procedures.
• Document client progress and maintain accurate records using Microsoft Word, Excel, and Adobe Reader.
• Transport clients to appointments and community activities as needed.
• Engage in social, recreational, wellness, and educational activities with clients.
• Communicate effectively with healthcare professionals, clients, and families.

Requirements

• High School Diploma or GED.
• Experience with physical and cognitive impairments/disabilities.
• Experience with Traumatic Brain Injury and Post-Traumatic Stress Disorder.
• Veteran or military experience preferred.
• Valid Driver's License and reliable transportation.
• CPR/AED/First Aid Certification from American Red Cross or American Heart Association.

Nice-to-haves

• Experience in community support or social services.
• Familiarity with HIPAA compliance and documentation standards.
