About the position

The Community Support Specialist plays a crucial role in providing integrated, holistic, community-based support services to individuals facing challenges related to psychiatric, physical, and substance use disorders. This position is essential in addressing the activities of daily living and managing symptoms associated with these disorders, ensuring that clients receive the compassionate care they need. The specialist will work in accordance with program standards and the 8 Dimensions of Wellness, which guide the delivery of services. In this role, the Community Support Specialist is responsible for completing services as outlined in the expectations worksheet and program manual, ensuring that all supporting documentation is completed in a timely manner, typically within 72 hours. Maintaining accurate and up-to-date client medical records is a critical aspect of this position, as is providing referrals, advocacy, and education tailored to meet the individual needs of clients. The specialist will also conduct appropriate risk assessments and facilitate access to necessary interventions, ensuring that clients receive comprehensive support. Transportation for clients may be required, and the specialist will monitor client hospitalizations, participating in discharge planning as needed. This position is vital in helping clients navigate their recovery journey and achieve their wellness goals, contributing to the overall mission of Compass Health Network to inspire hope and promote wellness in the communities served.

Responsibilities

  • Provide services to address activities of daily living, including the management of symptoms associated with psychiatric, physical, and/or substance use disorders per program standards and the 8 Dimensions of Wellness.
  • Complete services consistent with the requirements outlined in the expectations worksheet and program manual with the supporting documentation completed in a timely manner (within 72 hours).
  • Maintain all assigned client medical records to program standards.
  • Provide referrals, advocacy, information, and education to meet individual needs of clients.
  • Provide appropriate risk assessments and facilitate access to needed interventions as indicated.
  • Provide transportation for clients as needed.
  • Monitor client hospitalization and participate in discharge planning and follow per program standards.

Requirements

  • High School/GED required.
  • Associate's/Bachelor's/Master's degree in a human service-related field preferred.
  • At least four (4) years of experience required if no degree has been obtained, or a combination of higher education and experience if a High School/GED is the highest education obtained.
  • At least two (2) years of experience required if an Associate's degree is the highest education obtained, unless the degree is an Associate's of Applied Science in Behavioral Health Support from an approved institution, in which case no additional experience is required.
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves
