About the position

As a Community Support Specialist at Mercedes Diaz Homes Inc., you will play a vital role in supporting individuals with intellectual and developmental disabilities in their homes and communities. This position is centered around providing compassionate care and assistance to program participants as they engage in daily routines and recreational activities. Responsibilities

  • Conduct intake assessments for prospective new individuals and their families to determine needed services.
  • Coordinate services and organize schedules and appointments for individuals.
  • Maintain open lines of communication with key stakeholders, including individuals, parents, conservators, agencies, and employers.
  • Complete daily case notes and quarterly progress reports, sharing recommendations for individual service plans.
  • Advocate for individuals in court proceedings as part of the Judicial Intervention and Diversion Compliance Program.
  • Assist in building community partnerships to enhance available resources for individuals and their families.
  • Participate in required training, team, and company meetings to build knowledge, skills, and abilities.
  • Coach others in demonstrating the company's C.A.R.E. values and take ownership of personal behaviors.

Requirements

  • High school diploma or GED equivalent and at least 18 years of age.
  • Bachelor's degree in psychology, behavioral science, social services, or related discipline, or four years of relevant experience working with individuals with intellectual or developmental disabilities.
  • Current driver's license and California automobile insurance coverage for driving personal vehicle to community locations.
  • Ability to read, write, and speak English is required; bilingual in English/Spanish is preferred.

Nice-to-haves

  • Experience working with individuals with behavioral challenges, dual diagnoses, or forensic involvement.
  • Strong communication skills and ability to effectively engage with families and individuals.
