About the position

The Community Support Specialist plays a crucial role in assisting clients with severe and persistent mental illness to achieve successful community living. This position utilizes an assertive case management outreach approach based on the Rehabilitation Model, providing essential services such as housing acquisition, intensive case management, psychiatric rehabilitation, and supportive counseling. The specialist is responsible for developing individualized treatment plans, monitoring client progress, and facilitating access to various community resources and services.

Responsibilities

  • Assess and monitor a client's adjustment to community living.
  • Monitor client participation and progress in organized treatment programs according to the client's individual treatment plan.
  • Participate in the development or revision of individualized treatment plans, including completion of the DLA-20.
  • Provide individual assistance to clients in accessing needed mental health services and accompany them to appointments.
  • Assess clients' level of risks, strengths, and weaknesses to evaluate preparedness for independent living.
  • Encourage medication compliance and provide education regarding medications and monitoring of side effects.
  • Assist clients in accessing public services, including financial and medical assistance and emergency needs.
  • Intercede on behalf of clients within the community to assist in achieving community adjustment.
  • Maintain contact with hospitalized clients and facilitate discharge planning.
  • Train and support clients in daily living skills, including budgeting and personal grooming.
  • Create personal support systems involving family members or significant others.
  • Encourage recovery efforts and promote client independence and responsibility.
  • Provide support to families in treatment planning and linking to services.
  • Develop and support wellness and recovery goals in collaboration with clients.

Requirements

  • Associate's/Bachelor's degree in a human service-related field preferred.
  • At least four years of experience or a combination of higher education and experience if a High School/GED is the highest education obtained.
  • At least two years of experience or a combination of higher education and experience if an Associate's degree is the highest education obtained.
  • At least one year of experience required if a non-human service bachelor's degree is the highest education obtained.
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves

  • Certification in CPR, First Aid, CPI, and Medication Level I Aide upon hire.
