About the position

The Community Support Specialist at Compass Health Network plays a crucial role in providing integrated, holistic, community-based support services to individuals facing mental health challenges. The specialist will utilize a motivational, stage-matched approach to deliver individualized and group education services tailored to the strengths, needs, abilities, and preferences of clients and their families. The focus is on supporting clients in their journey toward recovery and resiliency goals, ensuring that services are delivered from a holistic perspective that incorporates the Eight Dimensions of Wellness. In this role, the Community Support Specialist will assist clients in identifying their strengths and needs related to substance abuse and mental illness, participating actively in the development of personalized Wellness Plans. The specialist will support clients in achieving the goals outlined in these plans, regularly reviewing and updating them as necessary to reflect the clients' progress and changing objectives. Additionally, the specialist will aid clients in accessing essential public services, including financial assistance, housing, and emergency support, ensuring they have the resources needed for their recovery. The position also involves maintaining contact with clients who are hospitalized, participating in discharge planning, and collaborating with all involved providers to develop comprehensive aftercare plans. Education and skill-building will be provided in various areas, such as daily living skills, employment and education, symptom management, and budgeting, with the ultimate goal of fostering independence in clients. The Community Support Specialist will also assist clients in developing personal support systems, working with family members, legal guardians, and recovery communities to enhance their support networks. This role may require accompanying clients to appointments and following up on their progress, ensuring they receive the necessary care and support.

Responsibilities

  • Provide individualized and group education services based on clients' strengths, needs, abilities, and preferences.
  • Assist clients in identifying their strengths and needs regarding substance abuse and mental illness.
  • Participate in the development of Wellness Plans for clients.
  • Support clients in achieving goals identified on Wellness Plans and review them as needed.
  • Aid clients in accessing public services such as financial assistance, housing, and emergency support.
  • Maintain contact with hospitalized clients and participate in discharge planning.
  • Provide education and skill-building in daily living skills, employment, symptom management, and budgeting.
  • Assist clients in developing personal support systems, including family and recovery communities.
  • Accompany clients to appointments to address medical or health needs.
  • Follow up with clients regarding appointments and completion of necessary forms.

Requirements

  • Experience in providing community-based support services.
  • Knowledge of mental health and substance abuse issues.
  • Ability to develop and implement individualized Wellness Plans.
  • Strong communication and interpersonal skills.
  • Experience in facilitating group education and skill-building sessions.

Nice-to-haves

  • Certification as a Qualified Mental Health Professional (QMHP).
