About the position

The Community Support Specialist at Health Connect America plays a vital role in providing mental and behavioral health services to children, families, and adults. This position involves case management, connecting clients with community resources, and ensuring compliance with state regulations. The specialist will work closely with clients, attend treatment team meetings, and provide on-call support as needed, all while fostering meaningful connections to help individuals achieve their potential in a supportive environment.

Responsibilities

• Provide quality case management services and ensure all documentation meets standards and requirements.
• Assist in marketing new referrals and communicate with customers and colleagues to address questions and concerns.
• Make two contacts per week with clients if no wraparound facilitator is present, documenting interactions accordingly.
• Connect families with necessary community resources and follow up to document the success of referrals.
• Meet with clients weekly or bi-weekly for up to 12 months as per program requirements.
• Attend and participate in weekly Treatment Team meetings.
• Serve as a liaison with other professionals and agencies related to the client.
• Complete a recovery support plan with all individuals within the first 30 days of referral to services.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with all state regulatory bodies.

Requirements

• Bachelor's degree in a human services discipline such as Social Work, Psychology, Sociology, Counseling, or Criminal Justice.
• Experience working with children and families in a case management or community resource position.
• Must be provisionally credentialed as a PCSS in Mississippi with eligibility for full credentialing as a CSS.
