About the position

The Community Support Specialist role at Compass Health Network is a full-time position focused on providing compassionate care to adolescents and families affected by substance use and mental health challenges. The specialist will engage clients through individual, family, and group therapy, as well as community-based projects, to facilitate positive change and support recovery goals. This position requires a commitment to holistic service provision and collaboration with an integrated health team.

Responsibilities

  • Provide individualized and group education services based on clients' strengths, needs, and preferences.
  • Assist clients in developing Wellness Plans and achieving their recovery goals.
  • Support clients in accessing public services such as financial aid, housing, and emergency assistance.
  • Maintain contact with hospitalized clients and participate in discharge planning.
  • Provide education and skill-building in daily living skills, employment, symptom management, and budgeting.
  • Help clients develop personal support systems, including family and recovery communities.
  • Assist clients in accessing needed services and accompany them to appointments as necessary.
  • Follow up with clients regarding appointments and documentation requirements.
  • Re-engage clients who have unexcused absences from the program to promote recovery efforts.
  • Complete required documentation and paperwork in a timely manner.
  • Communicate with integrated health team members regarding client wellness and treatment needs.

Requirements

  • High School diploma or GED required; Associate's or Bachelor's degree in a human service-related field preferred.
  • At least four years of experience working with individuals with psychiatric, substance use, or developmental disabilities if no degree is obtained.
  • At least two years of experience required if an Associate's degree is the highest education obtained.
  • At least one year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • Valid driver's license and minimum automobile coverage required.

Nice-to-haves

  • Experience in a clinical setting with adolescents and families.
  • Knowledge of community resources and services for mental health and substance use treatment.
  • Skills in motivational interviewing and holistic wellness approaches.
