About the position

The Community Support Specialist plays a crucial role in supporting individuals with mental health and physical health needs by developing and implementing personalized habilitation plans. This position focuses on therapeutic rehabilitative skill development, crisis intervention, and coordinating services to promote community integration and restoration of functioning. The role requires maintaining a productive schedule with a significant amount of face-to-face contact with consumers.

Responsibilities

  • Contribute to the development and implementation of individualized habilitation plans for consumers.
  • Monitor and update consumers' habilitation plans as needed.
  • Implement therapeutic rehabilitative skill development and Illness Management and Recovery (IMR) principles.
  • Coordinate and manage mental health and physical health services for consumers.
  • Provide crisis intervention as necessary.
  • Ensure active rehabilitation and support services to help consumers achieve community integration.
  • Maintain own appointment schedule and meet work productivity requirements, including 60% face-to-face contact weekly.
  • Operate an Agency vehicle and maintain an acceptable driving record.

Requirements

  • Bachelor's Degree from an accredited college or university in a helping profession (social work, counseling, psychiatric rehabilitation, or psychology).
  • If the degree is in a field other than a helping profession, a minimum of two years' professional experience in a community-based behavioral health setting is required.
  • Certified Psychiatric Rehabilitation Practitioner (CPRP) may substitute for one year of experience.
