About the position

The Community Support Specialist at AHL Healthcare Group Inc. is responsible for managing and training staff to provide care for adults with disabilities, promoting independence and community support. This full-time position requires a compassionate leader who can advocate for clients while ensuring compliance with relevant laws and regulations.

Responsibilities

  • Develop and ensure service plans for each participant, with measurable and observable progress.
  • Assure safety, health, and proper supervision of residents according to individual and program abuse prevention plans.
  • Organize daily schedules for the day program, meeting all licensing requests and requirements.
  • Demonstrate knowledge of the Vulnerable Adult policy and ensure staff compliance.
  • Report any misconduct or abuse to the Director of Services or appropriate authorities within 24 hours.
  • Maintain professional contact with residents' case managers, guardians, and other service providers.
  • Administer medication and document services provided daily in electronic records.
  • Engage with residents in social activities such as games, movies, and outdoor activities.
  • Provide daily documentation of services and monitor direct support staff's documentation.
  • Conduct screenings and tours for new referrals and assess placement appropriateness.
  • Complete necessary paperwork for county reimbursements and follow up on outstanding balances.
  • Schedule and lead monthly program staff meetings and trainings.
  • Maintain confidentiality and perform other duties as assigned.

Requirements

  • Bachelor's degree in a field related to human services and 1 year of full-time work experience, or
  • Associate's degree in a field related to human services and 2 years of full-time work experience, or
  • Diploma in a field related to human services from an accredited institution and 3 years of full-time work experience, or
  • Minimum of 50 hours of education and training related to human services and disabilities, along with 4 years of full-time work experience under qualified supervision.
  • Experience with Minnesota Statutes Chapters 245A, 245C, and 245D.
