The Community Support Specialist position at Simple Steps, LLC is a part-time role focused on enhancing the quality of life for Veterans with neurological disorders, physical disabilities, and traumatic brain injuries. The Community Support Specialist will work closely with Case Managers and family members to deliver dynamic social, recreational, wellness, educational, and individualized life skills training and activities. The role requires a commitment to supporting clients through effective care plans and ensuring that resources are utilized efficiently to meet the unique needs of each Veteran. In this role, the Community Support Specialist will be responsible for orienting and educating clients and their families about healthcare services and procedures. They will also be tasked with maintaining accurate documentation and providing timely data for invoicing purposes. The position requires travel to clients' homes, and the ability to transport clients during visits is essential. The Community Support Specialist will need to communicate effectively with healthcare professionals, clients, and families, both verbally and in writing, to ensure a collaborative approach to care. The position offers flexible scheduling, with up to 10 hours of work per week, with the potential for increased hours based on the needs of Veterans. The Community Support Specialist will be expected to track SMART goals and document progress weekly using Microsoft Word, Excel, and Adobe Reader.
