About the position

The Community Support Specialist position at Simple Steps, LLC is a part-time role focused on enhancing the quality of life for Veterans with neurological disorders, physical disabilities, and traumatic brain injuries. This position is designed for individuals who are passionate about providing high-quality home and community services to those in need. The Community Support Specialist will work closely with Case Managers and family members to deliver dynamic social, recreational, wellness, educational, and individualized life skills training and activities. The role requires a commitment to supporting clients through effective care plans and ensuring that resources are utilized efficiently to meet the unique needs of each Veteran. In this role, the Community Support Specialist will be responsible for orienting and educating clients and their families about healthcare services and procedures. They will also be tasked with maintaining accurate documentation and providing timely data for invoicing purposes. The position requires travel to clients' homes, and the ability to transport clients during visits is essential. The Community Support Specialist will need to communicate effectively with healthcare professionals, clients, and families, both verbally and in writing, to ensure a collaborative approach to care. The position offers flexible scheduling, with up to 10 hours of work per week, with the potential for increased hours based on the needs of Veterans. The Community Support Specialist will be expected to track SMART goals and document progress weekly using Microsoft Word, Excel, and Adobe Reader. This role is ideal for individuals who can work independently, manage their time effectively, and are committed to making a positive impact in the lives of Veterans.

Responsibilities

  • Support and carry out quality client care plans for Veterans.
  • Engage effectively with resources to meet client needs.
  • Orient and educate clients and their families about healthcare plan services and procedures.
  • Ensure accurate and complete documentation for client care.
  • Supply timely data for generating accurate invoices.
  • Travel to current or prospective clients' homes as needed.
  • Transport clients during visits when required.
  • Communicate with healthcare professionals, clients, families, and the general public through verbal and written communication.
  • Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader.
  • Implement patient care plans through home and community goal-related activities.
  • Assist with organization planning, cognitive awareness, doctor's appointments, and household management.

Requirements

  • High School Diploma or equivalent education.
  • Experience with physical and cognitive impairments/disabilities.
  • Experience with Traumatic Brain Injury and Post-Traumatic Stress Disorder.
  • Veteran or military experience is preferred.
  • Reliable transportation and a valid driver's license.
  • Agency-required auto liability insurance (100k/300k Bodily Injury).
  • Access to a private personal laptop or desktop computer for HIPAA compliance.
  • CPR/AED/First Aid Certification from American Red Cross or American Heart Association.

Nice-to-haves

  • Experience in community service or support roles.
  • Knowledge of healthcare services and procedures.
  • Ability to lift 50 lbs.
