About the position

The Community Support Specialist is responsible for assessing clients' needs and capabilities, providing counseling and assistance to ensure their overall well-being and successful integration into the community. This role involves establishing mental and medical health linkages, utilizing community resources, and supporting clients in developing independent living skills.

Responsibilities

  • Assess client's needs and capabilities.
  • Counsel and assist clients for overall well-being and treatment integration.
  • Establish mental and medical health linkages.
  • Utilize community resources to help clients attain their goals.
  • Provide support and guidance for independent living skills development.
  • Transport clients to appointments and activities as necessary.
  • Prepare daily contact notes and monthly progress reports for each client.
  • Serve as a liaison between clients and mental health professionals, family, and community agencies.
  • Participate in weekly Peer Group meetings to assess client activities and goal attainment.
  • Coordinate scheduling of client appointments as needed.
  • Monitor adequacy of entitlements and assist clients in accessing them.
  • Resolve crises in the best interest of the clients.
  • Communicate with support staff regarding client concerns or needs.

Requirements

  • Bachelor's degree (or equivalent) with at least 1-year experience in the mental health field preferred.
  • Two to three months working closely with a co-worker or supervisor.
  • Clean driving record.

Nice-to-haves

  • Ability to deal with crisis situations.
  • Ability to work with the deaf population and knowledge of deaf culture.
  • Fluency in American Sign Language.

  • Full-time position with a competitive hourly wage of $18.50.
