About the position

The Community Support Specialist at Compass Health Network plays a crucial role in providing integrated and holistic community support services for adults with long-term mental health disorders. This position involves working collaboratively with clients and their families to develop personalized wellness plans, assist with daily living skills, and connect clients with necessary resources. The role emphasizes recovery and resiliency, ensuring clients receive comprehensive support tailored to their individual needs.

Responsibilities

  • Use a motivational, stage-matched approach to provide individualized services based on strengths, needs, abilities, and preferences identified collaboratively with clients and families.
  • Assist clients in identifying strengths, needs, abilities, and preferences regarding mental illness/substance use and participate in the development of the initial wellness plan.
  • Provide services from a holistic perspective, incorporating coaching surrounding the Eight Dimensions of Wellness, focusing on both physical and mental health.
  • Make referrals to public services and additional providers as needed, including financial, housing, emergency assistance, and food.
  • Maintain contact with clients who are hospitalized, participate in discharge planning, and communicate with all involved providers in developing aftercare plans.
  • Provide education and skill-building in areas such as daily living skills, employment/education, symptom management, communication/relationships, budgeting, and maintaining a residence.
  • Assist clients with developing personal support systems, including work with family members, legal guardians, and recovery communities.
  • Meet service provision expectations per client needs and company guidelines, completing documentation and paperwork within the required timeframe.
  • Assist clients with transportation as clinically indicated, with driving being an essential function of the position.

Requirements

  • Associate's/Bachelor's/Master's degree in a human service-related field preferred.
  • At least four (4) years of experience in a relevant field if no degree is obtained, or a combination of higher education and experience if a High School/GED is the highest education obtained.
  • At least two (2) years of experience or a combination of higher education and experience if an Associate's degree is the highest education obtained.
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.
